Description

This screen collects the family history (Diagnosis ICD-10), as required for Meaningful Use Measures (MU) - MUM004v2.


Access 


Through the Side Bar section: Click on the Family History button on the Side Bar of the Progress Note screen. Can't find the Family History button: Click on Settings button on the toolbar of the Progress Note screen, Select the Side Bar Settings option. Select the Family History item from the available items area on the right side of the screen and drag/drop into the items window on the lower left corner of any Side Bar category. Click on the Save button in order to store the change. The Side Bar panel will refresh and the button will appear under the category where it was added.

Through the Search section: Click on theSearch button located at the left side of screen and then select the Family History button.


Sections

  • Add: Press theadd button to add a family members' information. (Add the family member in its corresponding category: Parents, siblings, Children, etc.)


  • Relationship: 
  • If you press the Add button, from the Parent section, the options will be as follows: Mother, Father. 
  • If you press the Add button, from the Siblings section, the options will be as follows: Sister, Brother.
  • If you press the Add button, from the Children section, the options will be as follows: Son, Daughter.
  • If you press the Add button, from the Aunts & Uncles section, the options will be as follows: Aunts, Uncles.
  • If you press the "Add" button, from the "Grand Parents" section, the options will be as follows: Grandmother, Grandfather.
  • First Name: Register the patient's family first name. 
  • Last Name: Register the patient's family last name. 
  • DOB: Patient's family date of birth.
  • Deceased [Yes/No]: Select yes or no.
  • Search: (Diagnosis: ICD-10)
  • Smoking Status: 
  • Edit: Press the pencil iconto edit data.
  • OK: 
  • Cancel:
