Description


The Patient Portal is a Web tool intigrated with an application for smartphones that could be downloaded through the Google Play for Androids and Apple Store for iOS platforms, with the final purpose to provide an easier and accessible experience for the patient.   



The new patient's portal, will replace the Microsoft HealthVault functions, as a tool to meet with the Measures:

  • MUCO07V2. PATIENT ELECTRONIC ACCESS (VDT) and the Measure: 
  • MUCO17V2. SECURE MESSAGING, as part of the CMS incentive program. 


Access

Go to www.mirecordclinico.com or download the Patient Portal application.


Main Screen

  • Email: 
  • Password: 
  • Forgot password?: Click on here, if password is forgotten. 
  • Submit: Click on, to access to the patients portal. 
  • Create an account: Click on, to create a new patient's portal account. 


Sections

  • Messages: Section where all messages will be displayed. Both patient's messages, sent and received.  


  • Appointments: This section displays patient appointment events. You will be able to observe future appointments, history of all appointments and request appointments with the provider of your choice.
  • Upcoming: Displays all future patient appointments.


  • By clicking on the appointment, the patient will be able to see the detail of the appointment, the provider and office, the status, the type of appointment and the office phone number. The patient may cancel or confirm the appointment.


  • History: In this section, the patient's appointment history. The patient will be able to see if it was canceled or confirmed. 


  • By clicking on the appointment, you will be able to see the detail of it, the date and time of the appointment, the status, the office area, the type of appointment and the office phone number where the patient will be seen.


  • Requests: In this section, the patient can request appointments with the provider of their choice. To request the appointment, the patient must click on the "More" button, located at the top right of the screen and select the "Request Appointment" option. 


  • The screen will be displayed to perform the provider search to request the appointment. The patient will be able to search for the provider through the "Search", by city or specialty. 


  • If you are an existing patient of the provider, you will be identified.


  • If the patient selects a provider and the provider is not an existing patient, the following will be deployed:


  • Notifications: In this section, notifications are displayed.


  • Account: Section where all the information related to the patient's account is displayed.
    • Name: Patient's name.
    • Email: Patient's email. 
    • Direct Address: New patient portal email.
    • Legacy Direct Address: Microsoft HealthVault email. 
    • Change Password: Section where user can change password. 
    • Logs: Section where displays the record of times patient has seen, downloaded and resent a CCDA.
    • Message Logs: Displays the messages logs. 
    • Manage Authorized Users: Section to add an authorized person, to manage the patient's account. 
    • Appointments Logs: By pressing, displays al the patients appointments. 


Settings 

  • Manage Authorized Users: Section to add an authorized person, to manage the patient's account.


  • Change Password: When pressed, it will display a window, to change the password of the account.


  • Delete Account: By pressing, it allows you to delete the direct account, once it has been deleted, it cannot be recovered. 


  • Help: User's help section. 


  • Support: Section for user support.
  • Show Tutorial: When pressed, it will show a tutorial of the portal's functions.
  • Call Us: Contact number, of our staff to request assistance.
  • Email Us: Email, where you can send messages to our staff for assistance.
  • Third Party Licenses: Information related to the portal license will be displayed.  
  • Privacy Policy: The privacy policy of the portal will be displayed. 
  • Authorized Users: Deploys the list of authorized users to the patient portal. This is mostly used in the case of pediatric patients.


  • Log out: Pressing closes the session.
