Description

The screen is utilized to document and archive questionnaires that have been filled out by the patient.


Access 


Through the Side Bar section: To access the Evaluation Forms in the Progress Note screen, click on the 'Evaluation Forms' button located on the Side Bar. If the 'Evaluation Forms' button is not visible, click on the 'Settings' button on the Progress Note screen's toolbar, then choose 'Side Bar Settings'. From the 'Available Items' area on the right, select 'Evaluation Forms' and drag it to the 'Items' window in the lower left corner under any Side Bar category. Afterward, click 'Save' to apply the changes. The Side Bar will update, and the 'Evaluation Forms' button will now be displayed in the chosen category.

Through the Search section: Click on the Search button located on the left side of the screen, then select the Evaluation Forms button.

Through the Patient Dashboard  section: Select the Evaluation Form History link in the Patient Dashboard section.

Buttons


  • Add: Allows to add a patient's questionnaire. 
  • Frequent Questionnaires: Allows the selection of favorite questionnaires. 
  • Print: The system enables printing of the questionnaire located in the 'Current Questionnaires' section. You have the option to print it with or without the answers. If you wish to print the form along with the answers, please select the 'Include answers in report' option, which is situated at the bottom left side of the screen.
  • History: Users will have the ability to access the entire history of patient questionnaires. These can be viewed in PDF format, either with or without the responses included. (Must select the "Include answers in report" option.)


  • Save: Enables the registration of a patient's questionnaire or multiple questionnaires.
  • Previous: Allows to close screen.
  • Questionnaire Area: Displays the questionnaires. 
  • Results: Displays the title and results of the patient's questionnaires.
  • Current Questionnaires: It shows the questionnaire that is currently in use.
  • Include  answers in report: Choose an option to add the answers to the questionnaire.
  • Delete Questionnaire: Enables the deletion of a questionnaire located in the Current Questionnaires section.
  • Instructions: Upon pressing, the instructions for the chosen questionnaire will appear as set up in the maintenance area.
  • Answer Summary: Upon pressing, the summary of the chosen questionnaire will be shown as set up in the maintenance area.
  • Total: The questionnaire's total score will be displayed, contingent upon the design set up in the maintenance area.
    • For Example: Has Results Area.
    • For Example: Does not have Results Area.
  • When the provider exits the screen, the patient's questionnaire information, subject to their configuration, is displayed in the Assessment/Plan section of the Advanced Progress Note screen.


Depending on the users configuration, the questionnaire completed can appear in the Assessment / Plan sections.
