Description

Screen used to document and store questionnaires completed for the patient.


Access 


Through the Side Bar section: Click on the Evaluation Forms button on the Side Bar of the Progress Note screen. Can't find the Evaluation Forms buttonClick on Settings button on the toolbar of the Progress Note screen, Select the Side Bar Settings option. Select the Evaluation Forms item from the available items area on the right side of the screen and drag/drop into the items window on the lower left corner of any Side Bar category. Click on the Save button in order to store the change. The Side Bar panel will refresh and the button will appear under the category where it was added.

Through the Search section: Click on theSearch button located at the left side of screen and then select the Evaluation Forms button.

Through the Patient Dashboard section: Click on the Evaluation Form History link, located in the Patient Dashboard section. 


Sections


Buttons


  • Add: Allows to add a patient's questionnaire. 
  • Frequent Questionnaires: Allows the selection of favorite questionnaires. 
  • Print: Allows to print the questionnaire found in the Current Questionnaires section. They can be printed, with or without the answers. If you want the form with the answers, select the Include answers in report option, located at the bottom left side of screen.
  • History: User will be able to view all patient's questionnaires history. They can be viewed in PDF format, with or without the answers. (Must select the Include answers in report option.)


  • Save: Allows to register a patient's questionnaire or several. 
  • Previous: Allows to close screen.
  • Questionnaire Area: Displays the questionnaires. 
  • Results:  Displays the title and results of the patient's questionnaires. 
  • Current Questionnaires: Displays the current questionnaire added at the moment. 
  • Include  answers in report: Select option in order to add the questionnaires answers.  
  • Delete QuestionnaireAllows to delete the questionnaire found in the Current Questionnaires section. 
  • Instructions: When pressed, the instructions of the selected questionnaire will be displayed. (As configured in the maintenance area.) 
  • Answer Summary: When pressed, the summary of the selected questionnaire will be displayed. (As configured in the maintenance area.)
  • Total:  The total score of the questionnaire will be displayed. This section will depend on the design configured in the maintenance area. 
    • For Example: Has Results Area.
    • For Example: Does not have Results Area.
  • When the provider leaves the screen, depending on their configuration, the information of patient's questionnaire was filled out, appears on the Advanced Progress Note screen, in the Assessment/Plan section.



Depending on the users configuration, the questionnaire completed can appear in the Assessment / Plan sections.  See: Assessment / Plan settings
