Steps to follow to request appointments, through the patient portal. Providers must have a Direct address on file.


EHRez Patient Portal can be accessed through the www.mirecordclinico.com address or by downloading the smartphone application, through the Google Play for Android and App Store for iOS.


Patient Sends Appointment Request 

  • In the patient portal, go to the Appointments section.


  • The following screen will be displayed: 


  • Press the More button, located at the top right of the screen and select the Request Appointment option.


  • The following screen will be displayed:  


  • The patient will be able to search for providers through the Search. Once you type your first or last name, press the Enter button on your keyboard.


  • Providers can be searched through Cities. 


  • The following screen will be displayed: 


  • Providers can be searched through Specialties. 


  • The following screen will be displayed: 


  • Select the provider from which you want to request the appointment. If it is an existing patient of the selected provider, the message will be displayed.


  • When the patient search is performed, the following scenarios will be deployed:
  • You are an existing patient - It means that you are an existing patient of the selected provider or have had some message exchange through the portal. It is considered an existing patient, when the provider has attended the patient.
  • No matching patient record found - It means that you are not an existing patient of the selected provider, or does not accept new patients.
  • Accepts new patients - It means that the selected provider accepts new patients and that he or she is not an existing patient of the provider.
  • The following screen will be displayed:


  • A preferred date will be displayed. If it is not the date you want, press the Change button.


  • The calendar will be displayed. 


  • Once selected, press the Done button. 


  • The selected date will be displayed: 


  • Once selected, choose the preferred time of day, either in the morning or in the afternoon.


  • Once you have selected the date and preferred time, press the Request Appointment button, located at the bottom of the screen.


  • The following screen will be displayed: 


  • Press the Send Request button, in order to send the appointment request.
  • Press the Cancel button, to cancel the appointment request.
  • When pressing the Send Request button, the following message will be displayed:


  • The appointment request will appear in the Requests section. Until the office adds the appointment, it will be displayed as Waiting.


Office Receives Patient Appointment Request

  • The office will receive notification of the patient's appointment request.


  • The patient's appointment request will be displayed: 


  • Press the Schedule button.  


  • Automatically, the Appointments screen will be displayed, to add the requested appointment.


  • Select the time you want the appointment. Once selected, press the Save & Close button.


  • The following confirmation message will be displayed: 


  • Press the Yes button to save the appointment.
  • Press the No button to cancel it.
  • Once you press the Yes button, the appointment will be displayed automatically. 


  • Or press the See Date button.


  • The Appointments screen will be displayed, to add the appointment, according to the availability of the office.


  • Once the appointment has been registered, the patient will then receive a notification. Press the Notifications button.


  • The notifications screen will be displayed: 


  • Click on the notification. The following screen will be displayed:


  • Press the Cancel button to cancel the appointment. (The patient may cancel the appointment 48 hours prior to the appointment. If not, the patient will be advised to contact the office where the appointment was recorded.)
  • Press the Confirm button. 


  • The appointment will be confirmed automatically.


  • The patient can check the record of their appointments by pressing the Logs button, located at the top right of the screen.


  • The following screen will be displayed: 


  • To update appointment logs, press the Check New Logs button.
  • When the patient enters the patient portal again, it will be displayed as follows:
