There are several options to add a consent form. 


Option #1 

  • Click on the Patients button, in main menu. 


  • The following screen will be displayed


  • Select the patient to whom you want to add a consent form.
  • Once selected, click on the Documents button, located at the top of the screen.


  • The following screen will be displayed:


  • Click on the Consent Forms button, located at the top of screen. 

 

  • The screen with all the consent forms registered in system will be displayed.

Nota: To add a  consent form Template,  you need to go to Settings / Office / Template screen, select the main category in General and the category in Consent Form.

  • Select the consent form, in the left side of screen. 
  • Then, click on the Select button.


  • The following screen will be displayed:


  • Click on the Replace Keywords and Close button.

 IMPORTANT NOTE: If the document has a signature [Keyword] registered, remember to have a Signature Pad installed.

  • As the Template contains Keyword of patient's signature, the screen will be displayed, to take the patient's signature.
  • If the patient signed successfully, click on the OK button.
  • The provider's signature in this example, will be displayed automatically.

Note: In order for the provider's signature be displayed automatically, you must have your signature registered in the system and in the Default Provider field, on the Patient Details screen, the provider that will attend to that patient must be displayed or selected. Otherwise, the signature of the connected user, will be displayed.

  • When the system finishes replacing the Keywords, it will display the document on the screen:

  

  • The following confirmation message will be displayed:


  • Click on the Yes button, to store the changes.
  • Click on the No button, to cancel the process.
  • The following message will be displayed:


  • Click on the Yes button, to indicate that consent is an advanced guideline.
  • The document will be displayed on the right side of screen. 


Option #2

  • Click on the "QuickLinks" text, located at the top of the screen.
  • Select the "Patients" option.


  • The following screen will be displayed


  • Select the patient to whom you want to add a consent form.
  • Once selected, click on the Documents button, located at the top of the screen.


  • The following screen will be displayed:


  • Click on the Consent Forms button, located at the top of screen. 

 

  • The screen with all the consent forms registered in system will be displayed.

  • Select the consent form, in the left side of screen.
