EHRez Patient Portal, can be accessed through the www.mirecordclinico.com address or by downloading the smartphone app, through the Google Play for Android and App Store for iOS.
Steps for account registration in EHRez Patient Portal
- Go to the www.mirecordclinico.com address or download the Patient Portal app.
- Click on the Create an account link.

- Enter your personal email address account and then click on the SUBMIT button.

Go to the www.mirecordclinico.com address or download the Patient Portal app.
- Click on the Create an account link.
- A message will be displayed, indicating that you must verify your email address to continue with the registration process.

- Click on the link received via email.
- Screen will be displayed, requiring you to enter First Name, Last Name, Password and Password Confirmation, to complete the registration process. Once you have entered the requested data, click on the SUBMIT button.

Once registration is completed, the main menu of Patient Portal will be displayed.

- Select the ACCOUNT option, among the alternatives available in the portal menu.
- The assigned email address, will be in the Direct Address field.
Mobile Version – App downloaded through Google Play for Android or in the App Store for iOS.
- Click on the Menu icon, located at the top left of the screen.
- Select the ACCOUNT option, among the alternatives available in the portal menu.
- The assigned DIRECT email address, will be in the Direct Address field.
- On the Ehrez Patient Details screen, enter the Direct Address assigned in the Patient Portal in the Direct field.
