How the patient requests appointments, through EHRez Patient Portal?

Steps to follow to request appointments through the patient portal. Providers must have a registered Direct address. 


EHRez Patient Portal can be accessed through www.mirecordclinico.com or by downloading the smartphone app through Google Play for Android and App Store for iOS.

  • In the patient portal, go to the “Appointments” section.


  • The following section will be displayed: 


  • Click on the “More” button located at the top right of the screen and select the “Request Appointment” option.


  • The following section will be displayed: 


  • The patient will be able to search for providers through “Search”. Once the first or last name is typed in, press the “Enter” button on your keyboard. 


  • Through the town search, by pressing the “Cities” button. 


  • The following section will be displayed: 


  • Through the search for specialties, by clicking on the “Specialties” button. 


  • The following section will be displayed: 


  • Select the provider, to whom you want to request the appointment. If it is an existing patient of the selected provider, the message will be displayed. 


  • When the patient search is performed, the following scenarios will be displayed:
  • You are an existing patient - It means that he/she is an existing patient of the selected provider or has had some message exchange through the portal. It is considered an existing patient when the provider has seen the patient.
  • No matching patient record found - Means, you are not an existing patient of the selected provider, or he does not accept new patients. 
  • Accepts new patients - It means that the selected provider accepts new patients and that he/she is not an existing patient of the provider. 
  • The following section will be displayed: 


  • A preferred date will be displayed. If it is not the date you want, press the “Change” button. 


  • The calendar will be displayed for date selection. 


  • Once selected, press the “Done” button. 


  • The selected date will be displayed:


  • Once selected, choose your preferred time of day, either in the morning or in the evening. 


  • Once you have selected the date and time of your choice, press the “Request Appointment” button located at the bottom of the screen. 


  • The following message will be displayed: 


  • Press the “Send Request” button to send the appointment request. 
  • Press the “Cancel” button to cancel the appointment request. 
  • When you press the “Send Request” button, the following message will be displayed:


  • The appointment request will appear in the “Requests” section. Until the office adds the appointment, it will be displayed as “Waiting”.
