There are several option to create a new patient record.


Option #1 

  • Click on the Patients button, in Main Menu. 


  • The following screen will be displayed: 


  • Click on the Add button, located at top of screen. 


  • Enter the patients demographic information. All required fields are identified with a red asterisk (*).
    • The required fields are: 
      • Name
      • Gender
      • Race
      • Ethnicity
      • Civil Status
      • DOB
      • Birth Sex 
      • Mobile

      • Physical Address 

      • Primary Language

  • Once the patients demographic information has been registered, user can add a profile or id picture, in the Pictures section, located at the right side of screen. 
  • To check patients eligibility, press the Insurance tab, located at the right side of screen. 


  • The section will be displayed, in order to register the patients insurance information. 


  • Once the patient's insurance information is registered, click on the Eligibility button.


  • Once all patient's information is registered, click on the Save button. This will create a patient record number. 


Option #2

  • Click on the QuickLinks text, located at the top right of screen, in main menu.
  • Select the Patients option.


  • The following screen will be displayed: 


  • Click on the Add button, located at top of screen. 


  • Enter the patients demographic information. All required fields are identified with a red asterisk (*).
    • The required fields are: 
      • Name
      • Gender
      • Race
      • Ethnicity
      • Civil Status
      • DOB
      • Birth Sex 
      • Mobile

      • Physical Address 

      • Primary Language

  • Once the patients demographic information has been registered, user can add a profile or id picture, in the Pictures section, located at the right side of screen. 


  • To check patients eligibility, press the Insurance tab, located at the right side of screen. 


  • The section will be displayed, in order to register the patients insurance information. 


  • Once the patient's insurance information is registered, click on the Eligibility button.


  • Once all patient's information is registered, click on the Save button. This will create a patient record number. 

Option #3

  • Go to the Appointments screen.
  • Select the new patient from calendar. 


  • Once selected, the user has two options to create the record. Press the Patient Details button, located on the right side of the screen.


  • Or press the Information button, located in front of the patient's name and select the "Create Rec" option.


  • In both cases, the following screen will be displayed:


  • Select the patient's gender and date of birth. Then, select the Go to patient details checkbox.


  • Click on the Save button.

  • Automatically, the following screen will be displayed:

      • Civil Status

      • Birth Sex

      • Physical Address

    • Once the patients demographic information has been registered, user can add a profile or id picture, in the Pictures section, located at the right side of screen. 

    • If patient has a medial plan, click on the Insurance link, located at the right side of screen. The section will be displayed, in order to register the patients insurance information.
    • To check patients eligibility, press the Insurance tab, located at the right side of screen. 
    • The following screen will be displayed, in order to add the patient's insurance information. 
    • Once the patient's insurance information is registered, click on the Eligibility button.
  • Once all patient's information is registered, click on the Save button. This will create a patient record number.
