• Go to the Appointments screen.
  • Identify the new patient appointment.


  • The following screen will be displayed:


  • Select the New Patient option.  
  • Click on the Create Rec button, once the mouse is over patient.


  • Or select patient and then click on the Patient Details button, located at the top of the screen in the Side Panel section. 


  • The following screen will be displayed:
  • Select patients gender.
  • Select patients date of birth.
  • Then select the Go to patient details option and click on the Save button. 


  • The Patient Details screen, will be displayed. 
    • Important: Verify that patient's name is displayed in First area and patient's last name, is in the Last area in the 2nd Last field.
  • Complete the following required fields: Race, Ethnicity, Civil Status, Birth Status, Mobile, Physical Address and Primary Language.
  • To save changes, click on the Save button, located at the top of screen. 
  • To return to the Appointments screen, click on the Previous button.
